Request a QDRO or pension estimate
Use the service request form when you know you need a court order to divide retirement assets or an estimate of a pension's present value.
It collects case and plan details, billing instructions and permission to communicate with the people involved.
Colorado cases use the same form. Select Colorado as the case state.

What to have ready
For a QDRO
- Settlement agreement or draft retirement division language.
- For an account-based plan, the most recent account statement.
- Date of marriage and the cutoff date for dividing the retirement assets.
- Judgment, if entered.
We will confirm any other records needed for the particular plan.
For a pension value estimate
- If the participant has not retired: a pension statement showing the accrued benefit, meaning the pension earned so far.
- The participant's date of hire.
The request form collects the other case details. If the participant is already retired, tell us in the form so we can confirm the benefit records needed.
A pension value estimate does not require a judgment or an account-balance statement.
Estimates are $700 flat. Federal civilian pensions (FERS and CSRS) and military pensions are excluded.
